Renumeration: Early Break Salary Points 4-6, £27,713.92 to £29,435.12 per annum

Key Focus

To provide individual and bespoke support to young people/adults at risk of homelessness and signpost to appropriate services to meet the clients needs.

You will help to prevent homelessness and support people to create self-determined outcomes that will lead to sustained positive change through the creation of support plans, innovative interventions and a coaching style approach.

(The role is part of the wider Pathfinder work across Greater Manchester)

Key Deliverables

  • Deliver wrap-around, trauma informed support to young people within the community and their own independent tenancies

  • Prepare and develop individually tailored support plans, achieve outcomes in accordance with contractual Key Performance Indicators   

  • Perform robust assessments of referrals for the service to ensure the service is provided to those that meet the eligibility criteria

  • Maintain accurate, up to date records on the case management system against individual support plans, safety plans and outcomes information

  • Liaise with local specialist services to facilitate access to support including: locally based offers, Criminal Justice services, health services, employment and training agencies

  • Ensure that individuals have an understanding of financial management in preparation for independent living, including focus on rent payment and budget creation

  • Build relationships with landlords, housing agencies and local housing teams

  • Complete dynamic risk assessments and maintain own safety by following lone working procedures

  • Adhere to good practice Safeguarding procedures to ensure the wellbeing of vulnerable people.
